Following the completion of his work on 20th Century Fox's The Fault in Our Stars in 2014, director Josh Boone collaborated with his childhood best friend Knate Lee to create a comic book using panels from Chris Claremont and Bill Sienkiewicz's New Mutants comics to depict what a potential film trilogy based on those comics might look like.

In April 2017, the film began pre-production in Boston, Massachusetts, in preparation for filming to commence there. Location reconnaissance was conducted, notably at Medfield State Hospital, which had previously been utilised for Shutter Island shooting (2010).

In December 2017, it was announced that Nate Walcott and Mike Mogis, who had previously collaborated on Boone's films, would be creating the film's soundtrack.
